Where is the office located?
We are located at 8841 San Jose Boulevard, on the Northeast corner of the intersection of Baymeadows Road and San Jose Boulevard, in San Jose Center.
What should I bring to my first appointment?
Please bring the following with you to your first appointment:
- Printed Patient Information and Financial Policy forms (If you are unable to fill out the forms prior to your appointment, please arrive 20 minutes early to fill them out before your appointment)
- List of your current medications
- Valid Dental Insurance Card
- Photo ID
- Your smile 🙂
Where can I find the New Patient Information forms?
Please print and complete the New Patient Information forms by clicking the link below, and bring them with you along with a list of any medications you are currently taking. We strive to make every visit to our office as pleasant as possible. Please let us know how we can help you quickly feel at home in our office. How do I transfer to your practice?
Call your current dentist and ask for a copy of your dental records. If they prepare a physical copy you can bring them with you to your first appointment. Often times, they create a digital copy and they prefer to send them via email. If so, please give them our office email address: email@example.com.
What insurance plans do you accept?
We accept most dental and medical insurance plans. Please call us at (904) 448-0441 for information about your particular insurance benefits.
What will I have to pay out of pocket?
Most insurance plans have an annual deductible and some also have annual maximum benefits. In many cases we are able to provide you with an accurate estimate of your out of pocket expenses. Please call us at (904) 448-0441 for information about your particular insurance benefits.
Can I pay my bill online?
Yes! We accept online payments through PayPal using Visa, MasterCard, American Express or Discover. You do not need a PayPal account to pay your bill online. Please call us at (904) 448-0441 if you have any questions about paying your bill online.
